This paper sketches an account of public health ethics drawing upon established scholarship in feminist ethics. Health inequities are one of the central problems in public health ethics; a feminist approach leads us to examine not only the connections between gender, disadvantage, and health, but also the distribution of power in the processes of public health, from policy making through to programme delivery. The complexity of public health demands investigation using multiple perspectives and an attention to detail that is capable (...) of identifying the health issues that are important to women, and investigating ways to address these issues. Finally, a feminist account of public health ethics embraces rather than avoids the inescapable political dimensions of public health. (shrink)

This article examines the implicit promises of fairness in evidence based medicine , namely to avoid discrimination through objective processes, and to distribute effective treatments fairly. The relationship between EBM and vulnerable groups is examined. Several aspects of EBM are explored: the way evidence is created , and the way evidence is applied in clinical care and health policy. This analysis suggests that EBM turns our attention away from social and cultural factors that influence health and focuses on a narrow (...) biomedical and individualistic model of health. Those with the greatest burden of ill health are left disenfranchised, as there is little research that is relevant to them, there is poor access to treatments, and attention is diverted away from activities that might have a much greater impact on their health. Until about thirty years ago, the extent of disclosure about and consent-seeking for medical interventions was influenced by a beneficence model of professional behaviour. Informed consent shifted attention to a duty to respect the autonomy of patients. The new requirement arrived on the American scene in two separate contexts: for daily practice in 1957, and for clinical study in 1966. A confusing double standard has been established. 'Daily consent' is reviewed, if at all, only in retrospect. Doctors are merely exhorted (...) to obtain informed consent; they often minimise uncertainties about 'best' treatment and they feel duty-bound to provide patients with an unequivocal recommendation for action. 'Study consent' in a clinical trial is reviewed prospectively, and doctors are compelled by regulation to point out that there is insufficient evidence to make a rational choice between two compared treatments. It has been impossible to devise informed consent practices that satisfy, in full, the competing moral imperatives of respect for autonomy, concern for beneficence with emphasis on the value of health, and a vigil for justice. A way must be found to experiment with various discretionary approaches that would strike a realistic balance among competing interests. (shrink)

In this paper, I discuss the pros and cons of the movement and philosophy of Transhumanism, with a focus on the concept of nothingness. I argue that all hitherto concepts of nothingness, both Western and Eastern, are inadequate for an understanding of the present technological position humans now find themselves in.

Objective: To review the principles and practice of the use of conscious sedation for IVF.Design: The pertinent literature was reviewed and recommendations are provided.Result(s): Conscious sedation appears to be the most commonly used method of pain relief for transvaginal retrieval of oocytes. Conscious sedation does not require the presence of an anesthesiologist and can be done in freestanding clinics. Agents commonly used include opioids in combination with benzodiazepines. This combination minimizes pain, decreases anxiety, and provides sedation and some amnesia Adjuvants (...) such as promethazine and hydroxyzine can also be used but often are not needed. Conscious sedation is well tolerated by patients and does not require highly specialized equipment. However, there are specific safeguards that should be followed. Only a few toxicity studies have been performed, but they are reassuring because they have not found significant effects on fertilization or cleavage.Conclusion(s): Conscious sedation appears to be a safe and cost-effective method of providing analgesia and anesthesia for transvaginal retrieval of oocytes. The acclaimed American sociologist and cultural philosopher Philip Rieff gained great academic prestige with his thesis on the emergence of 'Psychological Man' in western culture and with his classic book, Freud: The Mind of the Moralist, published in 1959. In this work and the later The Triumph of the Therapeutic (1966) he not only offered a highly original interpretation of the work of Sigmund Freud, but critically evaluated the enormous influence of psychotherapeutic thinking on Western culture. However, Rieff's later work (...) on the theory of culture did not garner the same attention, and his most recent writings have received very little critical engagement. In Sociology and the Sacred, Antonius A.W. Zondervan sets out to remedy this neglect, arguing that Rieff's work is ripe for intellectual reconsideration. (shrink)

In this paper I argue that it is morally important for doctors to trust patients. Doctors' trust of patients lays the foundation for medical relationships which support the exercise of patient autonomy, and which lead to an enriched understanding of patients' interests. Despite the moral and practical desirability of trust, distrust may occur for reasons relating to the nature of medicine, and the social and cultural context within which medical care is provided. Whilst it may not be possible to trust (...) at will, the conscious adoption of a trusting stance is both possible and warranted as the burdens of misplaced trust fall more heavily upon patients than doctors. OBJECTIVES: To study and report the attitudes of patients and general practitioners (GPs) concerning the obligation of doctors to act for the good of their patients, and to provide a practical account of beneficence in general practice. DESIGN: Semi-structured interviews administered to GPs and patients. SETTING AND SAMPLE: Participants randomly recruited from an age and gender stratified list of GPs in a geographically defined region of South Australia. The sample comprised twenty-one general practitioners and seventeen patients recruited by participating GPs. (...) RESULTS: In practice, acting for the good of the patient not only accommodates the views of patients and GPs on expertise and knowing best, but also responds to the particular details of the clinical situation. Patients had a complex understanding of the expertise necessary for medical practice, describing a contextual domain in which they were expert, and which complemented the scientific expertise of their GPs. General practitioners identified multiple sources for their expertise, of which experience was the most significant. The role of the GP included responding to individual patients and particular clinical problems and ranged from the assumption of responsibility through to the proffering of medical advice. CONCLUSION: This study found that GPs acting for the good of their patients covered a variety of GP actions and patient preferences. Beneficence was not justified by presumed patient vulnerability or the inability of patients to understand medical problems, but furthered through a recognition of the different areas of expertise contributed by both parties to the consultation. Sober and Wilson demonstrate convincingly the fallacies of arguments for fundamental biological and psychological selfishness and establish the plausibility of both biological and psychological altruism. However, I suggest that they are more generous to proponents of fundamental selfishness than they need be and that morality is closer to our evolved and learned capacities than they suggest. I am less generous toward advocates of fundamental selfishness than are our altruistic authors.

There is a widespread belief, more often implied than explicitly asserted, that Hume considered all necessary propositions to be analytic.Of course Hume did not use the analytic-synthetic distinction explicitly. This only come to the forefront with Kant; and it is Kant who is probably the main source of the above-mentioned belief. Kant ascribed to Hume the view that mathematical propositions are, in his terminology, analytic. If this is correct, then since mathematics was for Hume the paradigm of a body of (...) necessary truths, it is plausible to infer that he considered necessary propositions to be one and all analytic. The absence in Hume's writings of the analytic-synthetic terminology is by no means decisive: though he did not have the terms he may well have possessed the concepts, as evidenced by his use of them. How are we to understand Aristotle's famous doctrine of the mean? "If ten pounds are too much for a particular person to eat and two too little, it does not follow that the trainer will order six pounds"... In fact, the relation of morality to physical health is more intimate than mere analogy. Emotions involve a bodily process (cp On the Soul 403al6ff): for example, 'Anger is productive of heat' (On the Parts ofAnimals 650b35), while 'Fear is, indeed, a kind (...) of chill' (Rhetoric 1389b32). The hot temper of youth and the cool temper of old age, to take two extremes, are corollaries of physical extremes, of literal heat and cold respectively (Rhetoric 1389a2ff). In general, moral as well as bodily excellences are supervenient upon physical states; specific to moral excellence is what it supervenes upon, viz bodily pleasures and plains (Physics 246b3-247a20, an early passage not relevantly superseded). Health and virtue are both grounded upon the physiological condition of a psycho-physical entity... Yet it is hard to view the physiology of virtue as more than a general precondition. It more satisfactorily identifies natural virtue than virtue proper. The blood of young men is heated by nature (as though by wine), so that they are hot-tempered and hopeful; this makes them less timorous and more confident, and so more courageous, than the old (Rhetoric 1389al8ff). But the properly brave man is one 'who faces and who fears the right things and from the right motive, in the right way and at the right time' (11 5b1b7ff). (shrink)

The many well-publicized food scandals in recent years have resulted in a general state of vulnerable trust. As a result, building consumer trust has become an important goal in agri-food policy. In their efforts to protect trust in the agricultural and food sector, governments and industries have tended to consider the problem of trust as merely a matter of informing consumers on risks. In this article, we argue that the food sector better addresses the problem of trust from the perspective (...) of the trustworthiness of the food sector itself. This broad idea for changing the focus of trust is the assumption that if you want to be trusted, you should be trustworthy. To provide a clear understanding of what being trustworthy means within the food sector, we elaborate on both the concept of trust and of responsibility. In this way we show that policy focused on enhancing transparency and providing information to consumers is crucial, but not sufficient for dealing with the problem of consumer trust in the current agri-food context. In 1768 H. B. de Saussure studied chemistry with Baumé in Paris, and subsequently, using precise quantitative methods, he analysed minerals collected during his alpine journeys. He began to use the blowpipe in 1784, and later adapted it so that with a microscope and micrometer he could examine the effects of high temperatures on minute specimens of minerals. Analyses of air carried out with a portable eudiometer convinced him that air from alpine valleys contained more oxygen, and was therefore healthier, (...) than air from either mountain tops or plains. In 1788 he accepted the antiphlogistic theory after reading the annotated French translation of Kirwan's Essay on Phlogiston; he announced his conversion in a letter to the translator, Madame Lavoisier, whose reply is discussed and published here for the first time. The paper has two main aims. The first is to reformulate Hempel's version of the thesis of the symmetry of explanation and prediction, as regards the deductive covering-law model, so as to generalise it and make it no longer subject to some of the criticisms which have been directed at it (Section II). The second aim is to consider, with special critical reference to Hempel's recent treatment in Aspects of Scientific Explanation (New York and London, 1965), some central criticisms of (...) both the constituent parts of the above symmetry thesis, viz. that adequate explanations are potentially predictive (Section III), and that adequate predictive arguments are potentially explanatory (Section IV).
